Completion of this form does not guarantee admission. When openings occur, you will be contacted to complete the admissions process. A child entering Kindergarten must be 5 years old before August 1.
APPLICATION IS NOT CONSIDERED COMPLETE until the online application form has been submitted and the $50 application fee has been received by OLLIS. Additionally, if Catholic, parish proof of registration also needs to be turned in. This can be requested from your parish office.

Admissions Application
Catholic families applying must show proof of registration at Our Lady of Lourdes, Sacred Heart, or St. Thomas More Newman Center. Priority of admission is given to registered and active members of these three Columbia parishes.
